Why might you need an FCA?

Functional Capacity Assessments (FCAs) are more than just NDIS paperwork — they provide valuable insight into your daily needs, goals, and the supports that can help you live more independently.

A Change of Circumstances happens when there’s a significant shift in your life that affects the supports you need through the NDIS. This could include: - A new diagnosis - Changes in your health or disability - Moving to a new home or care setting - Changes in family or carer support - Increased safety concerns or support needs If your NDIS plan no longer reflects your current situation, you can request a plan reassessment by submitting a "Change of Details or Change of Circumstances" form, along with supporting evidence. At Redeem OT, we provide Functional Capacity Assessments to help support these changes. Our Telehealth assessments are convenient, comprehensive, and tailored to explain how your daily function has changed – and what supports are now required.

NDIS Change of
Circumstances

An NDIS Plan Review is an opportunity to look at how your current supports are working and whether you need any changes in your next NDIS plan. Most participants have a scheduled plan review every 12 months, but you can request one sooner if your needs have changed. During a plan review, the NDIA will consider: - How your current supports are helping you reach your goals - Whether your needs have changed over time - What new supports or services you might need - Any supporting evidence from professionals, like an occupational therapist At Redeem OT, we specialise in Functional Capacity Assessments (FCAs) completed via Telehealth, making the process easy and accessible. Our reports are clear, detailed, and tailored to each participant’s needs. They’re often used as key evidence for plan reviews, helping participants access the right supports and funding.

NDIS Plan Review

If you’re not yet an NDIS participant but are planning to apply, a Functional Capacity Assessment (FCA) can be a powerful piece of supporting evidence for your access request. The NDIA needs to understand how your disability impacts your daily life across areas like personal care, mobility, communication, and social participation. That’s exactly what an FCA does. At Redeem OT, we complete detailed Telehealth-based FCAs that clearly explain: - The nature of your disability - How it affects your ability to complete everyday tasks - What supports you currently rely on - Why ongoing and permanent support is needed Our reports are written in line with NDIS access criteria and can be submitted with your application to strengthen your case.

If you're applying for NDIS funding for equipment (like a wheelchair, shower chair, or sensory aids), or home modifications (like rails or ramps), an FCA can provide the functional evidence needed to support that request. How it Works

How It Works

The Functional Capacity Assessment (FCA) is designed to provide a thorough and personalised evaluation of your current level of function across all domains of daily life. The FCA aims to assist you in accessing the NDIS, obtaining additional funding, or adapting to any changes in your circumstances. During this process, you will work closely with an Occupational Therapist (OT), who will conduct a detailed assessment tailored to your unique needs. In collaboration with your relevant stakeholders, the efforts of the OT will help the NDIS gain insight into the support you need and how they can effectively assist you.

Appointment Scheduling

Your OT will call you to begin arranging the appointments. No long waitlists!

Assessment Sessions

This is when you and your OT will get to meet via video conferencing to conduct all necessary assessments and gather relevant information. Bring someone along with you!

Report Compliation

Your OT will be busy collating all information into a detailed report, including a thorough recommendations section. 

Report is Released

You’ll receive a copy of the final report, and we’ll also share it with any other parties you’ve agreed to include.
